Incident and Injury Report Form
Used by staff, mentors and host farms to record any injury, near miss or safety incident during a session.
Purpose
To capture incident detail accurately so that risk can be reviewed and reduced.
Scope
All sessions, farm visits and events.
When to complete this form
After any injury, illness, near miss, animal incident, property damage or behaviour incident — however minor.
Timeframe
Submit within 24 hours of the incident. Serious injury and child safety matters must be reported by phone immediately, before the form.
What to record
Date, time and location; who was involved and who witnessed it; what happened in sequence; first aid or treatment given; who was notified; and what has been done to prevent recurrence.
What happens next
The Operations Manager reviews every report, updates the site risk assessment where needed, and notifies guardians of any incident involving their child.
