Who completes this form

The parent or legal guardian of any participant under 18 years of age.

What you will need

Participant name and date of birth, your contact details, an emergency contact, and details of any medical condition, allergy, medication or accessibility need.

What you are consenting to

Participation in program and farm activities, first aid and emergency medical treatment if needed, and — separately and optionally — the use of photographs in Academy materials.

How to submit

Complete the consent step in your guardian portal, or download and return the printable form to the Governance Team before the first session.

Withdrawing consent

You may withdraw media consent at any time in writing. Withdrawing participation consent ends participation from the date we receive it.
